  PURPOSE OF THIS POLICY 

The purpose of this policy is to describe how we collect and use personal information about you, in accordance with the General Data Protection Regulation (GDPR), the Data Protection Act 2018 and any other national implementing laws, regulations and secondary legislation, as amended or updated from time to time, in the UK (“Data Protection Legislation”).

We currently collect and process the following information 

  • Personal identifiers, contact and characteristics for example name and date of birth

  • Correspondence and communication between your health team members

  • Our correspondence and communications with you

  • Information about inquiries or feedback

  • Information passed to us that is publicly available and relevant to your physiotherapy service

 

  COLLECTION OF PERSONAL INFORMATION 

We collect personal information that is necessary to allow us to provide a service to you. All personal information collected during assessment and treatment will be used only in relation to your physiotherapy care. The information collected is the minimum required to fulfil legal, accounting, taxation and contractual agreements to provide physiotherapy service. If you do not provide this information we may be unable to provide a service.

  USE OF PERSONAL DATA

Personal information is processed only where necessary to provide a service as agreed with you and to allow compliance with legal requirements. Personal information may be used for the service’s legitimate interests; however, this will not occur if those interests override any of your interests, rights and freedoms which require protection of personal information.

Personal information may be used for activities such as delivery of service, communication, record of service provision, processing of invoices and assisting in prevention and detection of fraud or corruption. This list is not exhaustive.

Where we need to use your personal information for another reason, other than for the reason we collected it, we will only use your personal information where that reason is compatible with the original purpose. Should it be necessary to use your personal information for a new purpose, we will notify you and communicate the legal basis which allows us to do so before starting any new processing.

  HOW PERSONAL INFORMATION IS STORED

Personal information is stored on a secure online platform and / or in a locked filing cabinet. It is only stored for as long as is necessary for the purpose for which it is collected in accordance with legal and professional obligations. When assessing what retention period is appropriate for your personal data, we take into consideration: 

• business and service requirements 

• any statutory or legal obligations

• the original data collection purposes 

• the lawful grounds on which we based our processing 

• the types of personal information we have collected

• the amount and categories of personal information.

 Use of all personal data is done using commercially reasonable and appropriate security measures.

 RIGHTS OF ACCESS, CORRECTION, DELETION AND RESTRICTION

  • Your right of access- You have the right to ask us for copies of your personal information 

  • Your right to rectification - You have the right to ask us to rectify personal information you think is inaccurate. You also have the right to ask us to complete information you think is incomplete

  • Your right to erasure - you have the right to ask us to erase your personal information in certain circumstances

  • Your right to object to processing - you have the right to object to the processing of your personal information in certain circumstances

  • Your right to data portability - You have the right to ask that we transfer the personal information you gave us to another organisation , or to you, in certain circumstances. 

You will not have to pay a fee to exercise any of the above rights. However, we may charge a reasonable fee if your request for access is clearly unfounded or excessive. Alternatively, we may refuse to comply with the request in such circumstances. 

Your identity will need to be confirmed prior to release of any personal data. This is to ensure that we keep information secure.

We use google analytics, which tracks anonymous data about how the site is being used and provide us with website statistics. My website contains widgets and badges which are associated with third parties tracking data and associated cookies. Unfortunately we are unable to offer a website which has no cookies, therefore if you do not consent to cookies you will not be able to use the site. You can remove any cookies put onto your computer from my website within your browser settings.

  RIGHT TO WITHDRAW CONSENT

You are able to withdraw consent at any time. On notification of your request, personal information will no longer be processed unless there is a legitimate legal requirement.

This policy was last updated on 29th January 2023. Any changes made to this policy will be provided to you in writing.The policy will be reviewed and amended annually or at points requiring change to practice.
